Oral antibiotic outpatient therapy vs. placebo in the treatment of uncomplicated acute appendicitis: a randomized double-blind placebo-controlled noninferiority trial APPAC IV (APPendicitis ACuta IV)

Acute appendicitis is one of the most common surgical emergencies worldwide. For over a century, surgical removal of the appendix has been considered the only treatment option with appendectomy still being one of the most common emergency surgeries. Landmark trials by our APPAC (APPendicitis ACuta) study group have shown that for imaging confirmed uncomplicated acute appendicitis, antibiotics are safe and effective also with only oral antibiotics and even placebo avoiding unnecessary surgeries resulting in major cost savings and decreased morbidity also allowing re-allocation of scarce hospital resources. APPAC IV randomized double-blind multicenter clinical trial compares oral moxifloxacin with placebo in an outpatient setting aiming to evaluate whether in addition to avoiding unnecessary surgeries, antibiotics and hospitalization or both can be omitted in the treatment of uncomplicated appendicitis further significantly increasing cost savings and patient satisfaction.
